Pond Medic Form-a-Lite 500ml

Introducing Form-A-LITE—an effective treatment against White Spot, Skin Flukes, Chilodenella, and most protozoan parasites.

Usage Instructions: Begin by switching off the UV sterilizer until the course of treatment is complete. Ensure the cap is tightened and the product is well mixed. Measure the appropriate dose using a clean watering can filled with pond water, adding Form-A-LITE with the provided measuring cup to achieve the correct dose. Slowly apply the solution to the pond over the course of one hour to prevent any sudden shocks to the pond ecosystem. Please note that Form-A-LITE should not be used with sturgeon, tench, or golden orfe. Additionally, do not use the product when the water temperature is below 12.5°C.

Dosage: Use 10ml per 682 liters (150 gallons) of pond water. Repeat the dose every 5-7 days, for a maximum of 3 doses.

Important Considerations:

• Accurately calculate your pond volume; avoid guessing. 

• Monitor your fish closely during all treatments. Avoid administering the treatment late at night, the day before your departure, or just before leaving the house.

• Ensure proper aeration during the treatment. If necessary, use an additional air pump.

• Maintain filtration throughout the treatment period.

• Turn off ultraviolet filters during treatment.

• Avoid overdosing, as it can lead to fish fatalities.

• Do not use in conjunction with any other medications.

• Do not use with salt.

Note: This product is intended for the treatment of ornamental pond fish only. It is strictly prohibited to allow fish treated by this chemical to enter the human or animal food chain.

Disposal:

• Do not dispose of the product in drains or watercourses. Empty the container completely and dispose of it safely. Discard the empty container in a dustbin. For unused product, dispose of it at a household waste site in its original container. Contact your local authority for further details.

Health & Safety:

• Avoid skin and eye contact. In case of contact, rinse thoroughly with plenty of water and seek immediate medical advice.

• Do not swallow.

• When using the product, wear suitable protective clothing and gloves.

• Keep out of reach of children and pets.

• Keep the bottle closed when not in use. Store between 2°C and 30°C (do not freeze).

• The product may stain skin and clothing.
